• Our care management ecosystem addresses the top four social conditions affecting patient health outcomes. We are seeking CNAs, PCAs, CMAs, RMAs, LPN,s and RNs to join our growing team of community health aide workers! This is not your traditional healthcare role. We are seeking Health Workers with the following:

      • Transport, assist, and care for clients going to medical appointments, errands, and recreational activities Serve as a patient advocate and healthcare liaison
      • Communicate ongoing care results and updates to relevant parties as appropriate
      • Collaborate with clients and families for best care opportunities as needed
      • Maintain a healthy and comfortable living environment by performing household chores/personal shopping & deliveryRequired Qualifications:
        • Well maintained and reliable personal 4-door vehicle/clean driving record
        • Healthcare certification- (ex. CNA); Available to work Mon - Fri 5am - 5pm
        • Willing to travel to all major metro hubs in Virginia (Charlottesville, Richmond, Virginia Beach, Norfolk etc.)
        • Previous experience in senior care or other related fields
        • Compassionate and caring demeanor, ability to build rapport with clients
        • Coachable, go-getter, entrepreneurial minded, self-starter and team player.
        • Must be experienced in updated technology for patient monitoring systems.
